Puberty

       

 Puberty वह अवस्था होती है जब female में secondary sexual characteristics का विकास होना प्रारम्भ हो जाता हैं तथा reproductive organs functional अवस्था में आ जाते हैं। puberty कि सामान्य उम्र 11/15 वर्ष के बीच होती है। puberty  के दौरान female में अनेक biological, morphological तथा psychological परिवर्तन होते हैं जो कि उसे लैंगिक रुप से परिपक्व बनाते हैं।

Convert childhood to adulthood puberty कहलाती है।


Puberty age- 

       

        Male -    12 to 16 year

       Female - 10 to 16 year


 Puberty के दौरान female में होने वाले परिवर्तन-

‌।       1-  Menarche एक female के जीवन में आने वाला first menstruation menarche कहलाता है।menarche की सामान्य उम्र 11से15 के बीच होती है।

       2- Ovulation‌ puberty के साथ ही female में ovulation की शुरुआत हो जाती है। Menarche के तुरंत बाद कुछ समय तक ovulation अनियमित रूप से हो सकता है परन्तु कुछ समय बाद यह नियमित हों जाता है।

       3-  Uterus fallopian tube or ovaries ये विकसित तथा परिपक्व होना प्रारम्भ हो जाते हैं।

         4- Breast ये आकार में बड़े तथा विकसित हो जाते हैं। Oestrogen नामक Hormone के कारण duct system का proliferation हो जाता है तथा breast में fat deposition प्रारम्भ हो जाता है।
    
       5- pubic or axillary hair विकसित होना प्रारम्भ हो जाते हैं।
       6- breast के साथ साथ hips में भी subcutaneous fat जमा होना प्रारम्भ हो जाता है।
        7- female की height बढ़ जाती है तथा pelvic wide हो जाती है।


 Puberty के five sign होते हैं-

  1 . Thelarchae
 2  . Adrenarchae
  3 .  Pubarchae
  4 .  Increase of height
  5.   Menarche

                                   ( First sign of puberty ) puberty का पहला sign

Note-    1- ( अगर 10 साल से पहले puberty आती है तो उसको precocious puberty बोलते हैं।) 

2- ( अगर 16साल के बाद puberty start होती है तो उसको delayed puberty बोलते हैं।

        Introduction- 

                                            एक    women में reproductive age 13/50  के दौरान 26/30 days  के अन्तराल में uterus से mucous तथा अन्य    पदार्थों से  mix‌ bleeding vagina के through होती है। इसको  menstrual cycle कहते हैं। 
  इसका Duration 3/5 days का होता है। एक women में first time menstruation को manarchae कहते हैं।  or 45/50 year में यह अपने आप समाप्त हो जाता है। जिसको menopause कहते हैं।
 एक Menstruation से दूसरे menstruation तक के duration को  menstrual cycle कहते हैं।इसका duration 26/30 days का होता है प्रत्येक women में  menstruation puberty से लेकर menopause तक monthly होती रहती है।

 Menstrual cycle- normal menstrual cycle‌ को eumenorrhoea कहते हैं।

Definition -

                           It is the visible manifestation of cycle physiological uterine bleeding due to shedding of endometrium out flow through hormone mainly H.P.O. axis.


           H- hypothalamus

           P- anti- pituitary gland 

           O- ovary 


         Hypothalamus से gonadotropin releasing hormone secret होता है। जो anti pituitary gland पे काम करता है। Antipituitary gland से दो hormone release होते हैं।  1- follicle stimulating hormone (FSH)  2- luteinizing hormone (LH)
release‌ होता है।

   ये दोनों hormone ovary पे work करते हैं। Ovary- oestrogen or progesterone Hormone को release करती है।

Normal MC-  

                     13 year menarche

                   .   Repeat in 28 days 
                   .   Range- 21-35 days 
                   .   1 year -  13 mences
                    .  15/45-. 30 year - 400 mences
                   .   Blood loss= 20 to 80 ml 
                  .   One mc में 35 ml/ mences 
                  .   Bleeding phase- 1 to5 days .
                  .   Bleeding duration- 2/7 days 

 Menstrual phases- 

     There are four phases

    1 proliferative phase/ follicle phase

    2  ovulatary phase 

    3 Luteal phase

    4 menstrual phase 


 1 proliferative phase/ follicle phase- 

Duration-14 days 

  जब 20 weeks का female fetus होता है तो उसमें 7 million follicles होते हैं। At birth 2 million बचते हैं। ये follicles each ovary में 2 million होते हैं । Puberty आने तक यह follicles घट कर 60000- 80000 यह जाते हैं, each ovary में। 
जब puberty start होती है। तो menstrual के time पर 6-8 follicles release होते हैं। इस  follicle को primary follicle कहते हैं ।इन‌ primary follicle पर hypothalamus gland or anti pituitary gland work करते हैं।

Hypothalamus gland- GnRH( Gonadotropin releasing hormone) secret करता है। 
 Anterior pituitary gland  दो‌ hormone secret करता है।
1. FSH ( follicle stimulating hormone)
2. LH   ( Luteinizing hormone)

FSH . Follicle stimulating hormone primary follicles का growth करता है।
Primary से secondary follicle बनाता है। और secondary से tersary follicle बनाता है। और tersary से Grafeum follicle बनाता है।
Grafeum follicle को mature follicle कहते हैं।  Grafeum follicle estrogen hormone release करता है। estrogen hormone uterus की endometrium layer को 0.2-3 mm तक thick करता है। 












  2 . Ovulatary phase- 

  
  इसका duration 14 days का होता है।

 अब Grafeum follicle पर  LH ( luteinizing hormone) work करेगा ।
जो Grafeum follicle बन चुका है। उसमें बहर की  covering को Grafeum follicle कहते हैं। और उसके अंदर primary oocytes होते हैं। अब 14 day को LH का contiontration बढ़ता है। और वो  Grafeum follicle को rupture कर देता है। अब primary oocytes divided होकर बहर आ जाते हैं। और ये secondary oocytes बनते हैं। ये fallopian tube में चले जाते हैं।

 3 . Luteal phase- 

   
  इस Grafeum follicle के जो  extra material है वो corpus luteum बनायेंगे ये corpus luteum progesterone Hormone secreat करता है।

 Progesterone को pregnancy hormone भी कहते हैं।

 ये Progesterone endometrium layer को thick करता है 6 mm तक ।
 ये Progesterone को pregnancy hormone इसलिए बोलते हैं क्योंकि ये endometrium को thick करता है और जो बाकी hormone को developed होने से रोकता है। और pregnancy में help करता है। corpus luteum की life 14 days  की होती है। और वो फिर degenerate हो जायेगा, luteal phase 14 days का होता है। corpus luteum जो कि yellow कलर का होता है जो degenerate होकर corpus albicons बनाता है। ये white colour का  structure होता है।











    4 . Menstrual phases-


Fertilization अगर नहीं हुआ तो corpus luteum degenerate हो जायेगा और जो endometrium की layer है वो shade हो जायेगी टूट जायेगी menstruation start हो जायेगा।
इसमें blood vessels vagina के through बाहर आ जाते हैं। 20 ml/ day 
Total flow - 40-80 ml 0-5 days तक।
